# FertPro Clinical Platform: specialty research and development blueprint

**Research snapshot: 23 September 2026 · Version 1.0 · Proposed product scope**

FertPro should be built as one clinical platform with a shared patient record and 23 distinct specialty workspaces. A robust specialty module needs its own clinical entities, measurements, workflow states and integration contracts; changing the heading on a generic consultation form is insufficient.

This blueprint translates primary-source research into an original product specification. Source-backed observations and manufacturer facts are identified with links. The detailed forms, workflows, acceptance tests and development priorities are **design recommendations**, not claims that every item is mandated by those sources or already implemented in FertPro. The supplied catalogue establishes module names, not clinical release readiness. No clinic deployment or device connector was audited for this report.

The 23 modules plus a shared Core explain the stated 24 platform catalogue entries. Diagnosis examples define data-model coverage, not an exhaustive diagnostic textbook. Measurements specify what to capture and its context, not universal diagnostic thresholds. Treatment algorithms, dosing, alert thresholds, reference intervals and local reporting requirements must be supplied and signed off by the appropriate clinical owners before deployment.

## What the research changes in the product design

- Reproductive care needs individual embryo/material identity and custody, beyond patient identity. ESHRE’s 2026 laboratory update explicitly addresses traceability and adds an embryo-biopsy section. [ESHRE](https://www.eshre.eu/Guidelines-and-Legal/Guidelines/Revised-guidelines-for-good-practice-in-IVF-laboratories)
- Antenatal, HIV and TB services can use WHO’s digital adaptation approach: data dictionaries, workflows, requirements and indicators linked to evidence. The publication date of a DAK must not be mistaken for proof that every embedded clinical rule is current. [ANC DAK](https://www.who.int/publications/i/item/9789240020306), [HIV DAK](https://www.who.int/publications/i/item/9789240085138), [TB DAK](https://www.who.int/publications/i/item/9789240086616)
- Pathology needs versioned reporting content. CAP released changes across 14 protocols on 16 September 2026; a fixed, unversioned checklist would age badly. Electronic content licensing must be checked separately. [CAP protocols](https://www.cap.org/protocols-and-guidelines/cancer-protocols/current-cancer-protocols/), [CAP FAQs](https://www.cap.org/protocols-and-guidelines/cancer-protocols/cancer-protocols-frequently-asked-questions/)
- Oncology exchange can build on mCODE 4.0.0, but that guide is not a complete prescribing or oncology workflow system. [HL7 mCODE](https://www.hl7.org/fhir/us/mcode/index.html)
- Modern equipment does not imply a public integration interface. GE publishes model/software-specific DICOM statements; Vitrolife describes an eWitness EMR API; surgical robot manufacturers describe digital ecosystems whose export rights still require confirmation. [GE conformance](https://www.gehealthcare.com/en/products/interoperability/dicom-conformance-statements/ultrasound), [eWitness](https://www.vitrolife.com/products/electronic-witnessing/ewitness/)

## Research method and limits

Research prioritised WHO, professional societies, HL7/IHE, regulators and manufacturer documentation over general articles. Guideline pages, publication summaries, technical documentation and selected current vendor/regulatory records were reviewed. Some full articles or PDFs were unavailable; the source register identifies those limitations. This is a broad architecture review, not a systematic review of every disease, a purchase comparison of every device, or verification of all paid standards/manuals.

Older sources are retained where useful for foundational workflow design; they are not described as the latest clinical guidance. US and UK sources inform design but do not establish local legal compliance, equipment availability or clinical policy. Each country/clinic implementation needs a locally approved terminology, reporting, privacy, prescribing and device-authorization profile. Do not advertise HIPAA, GDPR, ISO or regulatory certification on the basis of this document.

## 1. Shared Core: build once, use across specialties

| Core service | Required behaviour | Concrete release evidence |
|---|---|---|
| Clinic and identity | Tenant isolation; patient identifiers with issuer; duplicate review; audited merges/unmerges; related people without collapsing identities | Cross-tenant access tests; merge simulation with orders/results; guardian and donor permissions |
| Encounters and care teams | Appointments, queues, inpatient/outpatient episodes, locations, practitioner roles, handoffs and accountable tasks | State-transition tests and a complete end-to-end visit |
| Clinical record | Problems, history, allergies, observations, examination, diagnoses, plans and signed notes | Historical values retained; wrong-entry correction; signed amendment |
| Orders and results | Separate order, specimen, acquisition, result, verification and acknowledgement states | Cancel/recollect/repeat/amendment scenarios; critical-result escalation |
| Medicines | Formulary/product catalogue, medication reconciliation, prescribing, pharmacist verification, dispensing and eMAR | Unit/strength/route checks; partial dispense; held dose; adverse reaction |
| Consent and permissions | Purpose-specific consent; guardian/proxy rules; confidential record segments; break-glass with reason and review | Portal/export/print/search authorization tests, not just hidden UI elements |
| Documents and media | Signed reports, images, waveforms and video with original files, hashes, versioning and provenance | Retrieval and integrity checks; no public patient-media URLs |
| Scheduling and resources | Staff, rooms, theatre, equipment, procedure capacity, recalls and waitlists | Collision handling and cancellation/rebooking history |
| Inventory and finance | Product lots, expiry, stock movements, procedures, payer/billing and reconciliation | Clinical action does not disappear because billing is incomplete |
| Quality and governance | Incident register, clinical rule versions, audit, quality metrics and equipment maintenance | Named clinical owner; change approval; access/audit review |
| Integration | Interface registry, mapping versions, queues, acknowledgements, retries, reconciliation and monitoring | Duplicate/out-of-order/malformed/offline/replay tests |
| Continuity | Backups, tested restore, downtime workflows, export portability and operational monitoring | Restore drill and reconciled downtime encounter |

**Architecture recommendation:** extend the existing Django platform through bounded applications and shared services. Start with a modular application rather than creating 23 independently deployed systems. Enforce tenant access in data/query/API layers. Keep immutable source messages and signed reports; permit auditable corrections rather than silent editing. Store large clinical images/video in dedicated protected archives/object storage, with the EMR holding metadata and access-controlled references.

The marketing website and its enquiry system must remain separate from clinical data and device networks. Website hosting is not a PACS, clinical integration engine or robot gateway.

## 2. Reusable clinical data contracts

### Every diagnosis/condition

Store a stable condition ID, patient/subject ID, encounter/episode, clinical term, code system/version/code, recorded text, clinical status, verification status, onset/abatement, site/laterality, severity, evidence references, author and timestamps. Keep the following distinct: patient-reported history, clinician differential, confirmed diagnosis, rule-generated suggestion and billing code. Support more than one classification mapping without replacing the original clinical meaning.

Terminology candidates are SNOMED CT for clinical concepts, locally adopted ICD editions for reporting, LOINC for observations and UCUM for computable units. Selection requires licensed access where applicable and terminology review; this document deliberately does not invent exact codes. Add a mapping backlog for every local test, diagnosis, procedure, drug and device parameter before production exchange.

### Every measurement

| Field group | Proposed requirements |
|---|---|
| Identity | Observation ID; tenant; person or explicitly modelled fetus/material subject; encounter; specialty episode |
| Meaning | Clinical concept/code/version; body site/laterality; specimen; method; component relationships |
| Value | Numeric, coded, boolean, ratio, range or text as appropriate; comparator for values such as less than detection limit |
| Units | Original unit plus validated canonical unit; conversion formula/version; preserve original value |
| Time | Effective time/interval; device clock; receive time; entered time; timezone/offset and clock-quality flag |
| Interpretation | Applicable reference range and population/context; abnormal/critical flag with source; never assume missing is normal |
| Provenance | Device/model/software/serial; calibration/QC status when relevant; operator; imported/manual/patient-reported source |
| Lifecycle | Preliminary/final/corrected/entered-in-error; reviewer; amendment link; raw-source message/file reference |
| Derived values | Formula, software/version, inputs, rounding, reference chart and uncertainty/data-completeness flag |

The companion CSV contains **measurement groups**, not an exhaustive atomic database dictionary. Expand each group (for example BPD/HC/AC/FL) into individual coded fields with datatype, cardinality and validations during module design. A printed unit label is not enough: the engine must validate the unit against the particular analyte or parameter. Scores need component-level storage, language, instrument version, licensing and intended population.

### Relationships that must be first-class

Pregnancy → fetus → scan; IVF cycle → gamete/embryo → custody event; pathology case → part → block → slide; order → specimen/aliquot → assay → report; surgical case → implant/specimen/device exposure; oncology episode → regimen → cycle → administration; rehabilitation episode → goal → session → outcome.

FHIR R4 is a proposed exchange baseline where counterparties support it, not a claim that R4 is the newest release or that a FHIR-shaped database is mandatory. Use agreed profiles for Patient/RelatedPerson, Encounter, Condition, Observation, ServiceRequest, Specimen, DiagnosticReport, ImagingStudy, MedicationRequest/Dispense/Administration, Procedure, CarePlan, Consent, Device, Provenance and AuditEvent. Some reproductive and specialty relationships require carefully governed extensions or dedicated internal entities; do not force every embryo or fetus into an ordinary patient record without an agreed model. [FHIR Observation reference](https://hl7.org/fhir/R4/observation.html)

## 3. Specialty ownership and module boundaries

| Modules | Boundary and shared handoff |
|---|---|
| Gynae / MFM / IVF | Gynae owns non-pregnancy reproductive disease; MFM owns pregnancy/fetus assessments; IVF owns assisted-reproduction cycles and embryology. Share person-level history, with controlled participant access. |
| IVF / Cryobank | IVF creates/uses reproductive material; Cryobank owns storage location, authorization and custody. Share stable material IDs and event acknowledgements. |
| Laboratory / Pathology | Laboratory owns assay workflows; Pathology owns tissue/cytology lineage and diagnostic interpretation. Share orders, specimen identifiers and molecular results. |
| Imaging / Radiology | Imaging owns scheduling/acquisition/archive; Radiology owns interpretation, signed reports and finding communication. Other specialties may interpret their own studies within privileges. |
| Pharmacy / all specialties | Specialty teams define treatment intent; Pharmacy supplies prescribing verification, dispensing and administration services. High-risk regimens need specialty-specific checks. |
| Surgery / Oncology / Rehabilitation | A procedure is one encounter in a larger disease and recovery journey; link episodes instead of copying notes. |
| Paediatrics / Neonatology / MFM | Mother, fetus and newborn relationships need explicit transitions; newborns have separate identities, especially in multiple births. |

## 4. The 23 specialty specifications

Each specification includes workflow, diagnosis coverage, measurements, equipment, acceptance criteria, reporting and staged scope. All detailed requirements below are **proposed FertPro design**. Linked references provide clinical/technical foundations; they do not certify the entire proposed feature set.

### 01. FertPro Primary Care

- Preventive-care recalls, immunization administration with batch records, chronic disease follow-up, medication reconciliation and referral tracking share one timeline.

- Close the loop on ordered tests, reviewed results, patient communication and missed appointments; distinguish a referral sent from one actually completed.

**Diagnosis coverage and recording requirements**

Hypertension; diabetes; asthma/COPD; acute respiratory and urinary infections; musculoskeletal complaints; anaemia; obesity; multimorbidity. Capture suspected versus confirmed status, onset, severity, evidence, resolution and responsible clinician; symptom codes remain valid before diagnosis.

| Oocytes/embryos | counts and categorical grades | Individual IDs, maturity, pronuclei, day/hour and grading system |
| Embryo development timings | hours | Reference event, incubator clock and observation source |

**Equipment and integration requirements**

- Time-lapse incubator/EmbryoViewer → authorized embryo-linked exports; EmbryoScope 8 is a candidate, not a verified FertPro connector.

- Electronic witnessing → patient/cycle/sample identifiers and event acknowledgements; Vitrolife describes an EMR API, requiring contract and validation.

- Ultrasound, hormone analysers, CASA and laser/biopsy workstations → images/results/procedure records via appropriate vendor interface.

**Minimum acceptance scenarios**

- A mismatched person, dish or embryo must block the affected electronic transition and create an exception record.

- A failed interface cannot be marked witnessed; manual witnessing requires the approved fallback process and later reconciliation.

**Reports and indicators:** Fertilization rate with mature/inseminated denominator explicitly chosen; blastocyst formation by cohort; live birth per initiated cycle and per transfer reported separately with follow-up completeness.

**Initial release scope:** Cycle scheduling, monitoring, embryology lineage, witnessing records, transfer and outcome register.

### 05. FertPro Cryobank

**Distinct record:** Physical reproductive-material inventory with immutable custody events, consent scope and exact storage coordinates.

**Core workflows and forms**

- Accession material with owner/donor links, source cycle, type, infectious-risk handling and identity verification.

- Map tank → canister/rack → cane/box → position → straw/vial/device; record freeze protocol, operator, witness and contents.

- Manage storage consent, review dates, billing, quarantine and transfer authorization as separate states.

- Record every move, warming, use, shipment and disposition with source/destination, reason, two-person verification where required and full lineage.

**Diagnosis coverage and recording requirements**

This is principally a material-management service, not a diagnosis engine. Link fertility preservation, donor storage or treatment indication to the clinical record; distinguish quarantine status, test evidence and material suitability from a person’s diagnosis.

**Measurement groups**

| Measurement | Units / representation | Required context |
|---|---|---|
| Storage temperature | °C | Probe, calibration and tank location |
| Liquid nitrogen level | cm or % | Sensor type, measurement method and timestamp |
| Material quantity | count; mL where applicable | Item versus container count |

**Equipment and integration requirements**

- Tank sensors, oxygen/environment monitors, barcode/RFID readers and transport loggers → event stream through manufacturer gateway.

- Keep independent local audible alarms and emergency response; the EMR dashboard is not the sole safety system. RFID must be validated for cryogenic materials and physical layout.

**Minimum acceptance scenarios**

- No two active items may occupy the same exclusive slot; a failed relocation transaction preserves original custody.

- Consent expiry must create review work, never automatic disposal; temperature alert delivery and escalation must be tested end to end.

**Reports and indicators:** Inventory reconciliation discrepancies per audited item; alarm acknowledgement time; overdue consent reviews; capacity by usable validated storage positions.

**Initial release scope:** Inventory hierarchy, custody ledger, consent/review register and manual temperature logs.

**Advanced scope:** Sensor monitoring, transport logger import and electronic witnessing after physical validation.

**Research foundations:** [ESHRE good practice in IVF laboratories (2026)](https://www.eshre.eu/Guidelines-and-Legal/Guidelines/Revised-guidelines-for-good-practice-in-IVF-laboratories), [Vitrolife eWitness](https://www.vitrolife.com/products/electronic-witnessing/ewitness/)

- Phlebotomy and reception capture barcode, collector, transport conditions, acceptance/rejection and aliquot genealogy.

- Benches manage worklists, analyser runs, calibration, internal QC, reagent lots, reflex testing and repeats with explicit reasons.

- Authorized staff validate results, communicate critical findings, issue corrected reports and support microbiology organism/antimicrobial susceptibility structures.

**Diagnosis coverage and recording requirements**

Laboratories principally report tests and interpretations: haematology, chemistry, endocrinology, immunology, serology, microbiology and molecular diagnostics. Keep clinician diagnosis/indication separate from detected organism, abnormal analyte or laboratory interpretive comment.

**Measurement groups**

| Measurement | Units / representation | Required context |
|---|---|---|
| Haematology | g/L or g/dL; ×10⁹/L; fL | Analyte, specimen, method and population interval |
| Chemistry/electrolytes | mmol/L; mg/dL; µmol/L | Analyte-specific units, haemolysis and dilution |
| Coagulation | seconds; ratio; INR | Method, anticoagulant context and specimen |

| QC metrics | control value; SD; CV% | Control lot, level, run and rejection rule |

**Equipment and integration requirements**

- Chemistry, haematology, coagulation, immunoassay, blood gas, microbiology and PCR instruments → LIS/middleware, usually model-specific HL7 v2, ASTM-derived or vendor protocols.

- Orders may travel out and results return only where both directions are supported; portable POCT also needs operator competency and device QC information.

**Minimum acceptance scenarios**

- Failed QC must prevent routine auto-release for affected tests; replayed results must not duplicate observations.

- A corrected result must retain the previous value and reach the prior report recipients; unit mapping errors must quarantine the result.

**Reports and indicators:** Turnaround by collection/receipt/verification separately; specimen rejection per received specimen; critical-result acknowledgement; QC failures by run.

**Initial release scope:** Accessioning, specimen tracking, manual result validation, critical notification and quality logs.

**Advanced scope:** Validated analyser interfaces, constrained auto-verification and microbiology surveillance.

**Research foundations:** [WHO laboratory quality management handbook (2011)](https://www.who.int/publications-detail-redirect/9789241548274), [HL7 FHIR R4 Observation](https://hl7.org/fhir/R4/observation.html)

### 07. FertPro Pathology

**Distinct record:** Case → specimen part → tissue block → slide → stain/image → signed report and addenda.

**Core workflows and forms**

- Accession surgery/biopsy/cytology material with anatomical site, laterality, orientation, collection/fixation times and clinical question.

- Grossing records dimensions, margins, photographs, cassette mapping and processing; histology tracks recuts, special stains and immunohistochemistry.

- Pathologists use structured organ-specific reports with morphology, grade, invasion, margins, nodes and relevant biomarkers.

- Support consultations, multidisciplinary review, molecular testing, archival retrieval, slide loans, report amendments and digital-slide review.

**Diagnosis coverage and recording requirements**

Benign/inflammatory conditions, dysplasia and malignant neoplasms by anatomical site and histological subtype. Separate pathological diagnosis from clinical staging; record grading/staging system and edition, specimen adequacy and uncertainty.

**Measurement groups**

| Measurement | Units / representation | Required context |
|---|---|---|
| Specimen/tumour dimensions | mm or cm | Orientation, gross versus microscopic source |
| Margin distance | mm | Named margin, invasive/in-situ distinction |
| Lymph nodes | counts | Examined versus involved, station and deposits |
| Mitotic activity | count per defined area | Area in mm² and method |
| Biomarker expression | %; score; categorical result | Clone/platform, tissue block and scoring system |
| Fixation/cold ischaemia intervals | minutes or hours | Actual collection and fixation timestamps |
| Digital-slide resolution | µm/pixel | Scanner, objective, focus QC and image identifier |

**Equipment and integration requirements**

- Barcode printers/readers, tissue processors, stainers, slide scanners and molecular platforms → tracked production events and reports where interfaces exist.

- Whole-slide images stay in a suitable archive/viewer with specimen/block/slide linkage; DICOM WSI or vendor format/export must be checked per scanner. PDF alone loses searchable diagnostic structure.

**Minimum acceptance scenarios**

- Every slide must resolve to one block, specimen and patient; a re-labelled slide requires audited correction.

- Amended tumour staging or margin status must notify the treating team and retain original signed content.

### 20. FertPro Mental Health

**Distinct record:** Therapeutic episode with formulation, safety planning, longitudinal symptoms and carefully governed sensitive information.

**Core workflows and forms**

- Assess presenting concerns, psychiatric/medical history, mental state, substance use, social context and the patient’s own goals.

- Record diagnosis and formulation separately; build collaborative care and crisis/safety plans with named contacts and review dates.

- Track psychotherapy sessions, medication monitoring, inpatient/community transitions and multidisciplinary responsibilities.

- Support age-appropriate instruments, consent, safeguarding and disclosure restrictions without hiding essential emergency information from authorized clinicians.

**Diagnosis coverage and recording requirements**

Depressive, anxiety, bipolar, psychotic, trauma-related, substance-use and neurodevelopmental disorders; self-harm/suicide concerns as clinically assessed phenomena. Distinguish positive screening from diagnosis and avoid treating a risk score as a prediction of an individual act.

**Measurement groups**

| Measurement | Units / representation | Required context |
|---|---|---|
| Symptom scores | instrument/component score | Version, language, respondent and timeframe |
| Mental state | structured narrative/categorical | Appearance, cognition, thought, affect and context |

| Medication monitoring | kg; mmHg; laboratory-specific units | Drug-specific indication and schedule |
| Substance use | quantity/frequency | Substance, route, last use and source |
| Sleep/function | hours; named scale | Patient report, instrument and baseline |
| Treatment engagement | sessions; dates | Attendance, patient preference and reason for interruption |

**Equipment and integration requirements**

- Teleconsultation and patient questionnaires → consented records with identity checks; wearables are optional contextual data.

- ECT/rTMS systems, if offered, → treatment session parameters and reports through approved export; never remote energy control through a general EMR.

**Minimum acceptance scenarios**

- Sensitive note restrictions must be tested against portal, print, export, search and emergency-access paths.

- A questionnaire indicating acute concern must enter the locally approved human response pathway; automated scoring alone cannot close the case.

**Reports and indicators:** Follow-up after discharge, care-plan review, symptom/function change and engagement with declared populations; measure alert follow-through rather than claiming suicide prediction.

**Initial release scope:** Assessment, formulation, care/safety plans, session notes and medicines.

**Advanced scope:** Telehealth/questionnaire integration and specialty treatment records after access controls are proven.

**Research foundations:** [WHO mhGAP guideline, third edition (2023)](https://www.who.int/publications/i/item/9789240084278), [HL7 FHIR R4 Observation](https://hl7.org/fhir/R4/observation.html)

### 21. FertPro Rehabilitation

**Distinct record:** Function-centred episode linking impairments, activities, participation, goals and delivered interventions.

**Core workflows and forms**

- Physiotherapy, occupational therapy and speech/language teams assess baseline function, environment, restrictions and patient priorities.

- Set measurable goals, treatment schedule, assistive-device needs and home programme with multidisciplinary ownership.

- Record each session’s exercises/interventions, assistance, tolerance, progression and reasons for deviation.

- Reassess outcomes, equipment fit and caregiver training, then document discharge or ongoing maintenance plan.

**Diagnosis coverage and recording requirements**

Stroke, spinal cord injury, brain injury, cerebral palsy, musculoskeletal injury, postoperative impairment, chronic pain and cardiopulmonary limitation. Link underlying disease to functional problems; diagnosis alone does not describe rehabilitation need.

**Measurement groups**

| Measurement | Units / representation | Required context |
|---|---|---|
| Range of motion | degrees | Joint, side, active/passive and method |
| Strength | N; kg-force; named grade | Muscle/group, dynamometer and position |
| Walking performance | m; m/s; seconds | Named test, aid, surface and assistance |
| Balance/function | instrument score | Version, components and assessor |
| Pain/fatigue | scale score | Activity and timing |
| Exercise exposure | repetitions; minutes; resistance | Task, assistance and tolerance |
| Speech/swallow/function goals | instrument or structured observation | Method, consistency tested where relevant and specialist interpretation |

**Equipment and integration requirements**

- Dynamometers, gait/force platforms, wearable sensors, balance systems and therapy equipment → session-linked data.

- Robotic gait trainers/exoskeletons → manufacturer-approved session summaries, assistance parameters and completed work; therapist remains responsible and EMR does not command movement.

**Minimum acceptance scenarios**

- Change in test method or walking aid must be visible when comparing outcomes.

- Device-recorded activity must be confirmed against the intended patient/session; incomplete or aborted therapy must not count as delivered dose.

**Reports and indicators:** Goal attainment, function change, completed versus planned sessions, home-programme review and assistive-device delivery; account for baseline severity and follow-up loss.

**Initial release scope:** Multidisciplinary assessment, goals, sessions, outcome measures and home programme.

**Advanced scope:** Gait laboratory, sensor and rehabilitation robot data integration.

**Research foundations:** [WHO rehabilitation intervention packages (2023), neurological module and linked modules](https://www.who.int/publications/i/item/9789240071131), [HL7 FHIR R4 Observation](https://hl7.org/fhir/R4/observation.html)

**Diagnosis coverage and recording requirements**

HIV, TB, viral hepatitis, malaria, respiratory/urinary infections, invasive bacterial/fungal infections and opportunistic infections. Record suspected/probable/confirmed status, organism and resistance evidence; colonization and contamination must remain distinct from infection.

**Measurement groups**

| Measurement | Units / representation | Required context |
|---|---|---|
| Viral load | copies/mL or IU/mL | Assay, detection limit, specimen and collection date |
| CD4 | cells/µL; % | Method and longitudinal context |
| Microbiology susceptibility | MIC; category | Isolate, antimicrobial, method and breakpoint edition |
| Molecular result | qualitative/quantitative | Target, assay, resistance marker and interpretation |
| Weight/temperature | kg; °C | Treatment monitoring and clinical context |
| Treatment exposure | days; doses | Regimen/version, adherence source and interruptions |
| Programme outcomes | coded category | Disease-specific definition, assessment date and evidence |

**Equipment and integration requirements**

- PCR platforms, culture systems and susceptibility analysers → Laboratory integrations; maintain accession/isolate identifiers.

- Public-health registries/DHIS2 or national programme tools → only agreed datasets and permitted transfer; product name alone does not establish an available interface.

**Minimum acceptance scenarios**

- An antimicrobial resistance result must stay attached to the correct isolate and specimen.

- Restricted HIV/TB data must be tested through exports and shared dashboards; surveillance counts should use the approved de-identification rules.

**Reports and indicators:** Treatment completion, viral-load monitoring, time to organism/result review and antimicrobial review completion; choose programme-approved cohorts and exclude unresolved outcomes explicitly.

**Initial release scope:** Infection history, evidence, treatment/adherence, precautions and follow-up.

**Advanced scope:** Microbiology connectivity, programme exchange and stewardship dashboards.

### 23. FertPro Diabetes and Metabolic Care

**Distinct record:** Longitudinal metabolic record combining laboratory control, device-generated glucose, complications and patient goals.

**Core workflows and forms**

- Classify diabetes and metabolic conditions, capture onset, treatment history, nutrition, activity and relevant psychosocial context.

- Review glucose patterns alongside medicines, meals, illness and hypoglycaemic events; record shared goals and education.

- Track renal, eye, foot, cardiovascular and neuropathy assessments with referrals and overdue review worklists.

- Support obesity/metabolic care, connected-device onboarding, data-sharing consent and transitions such as pregnancy or paediatric-to-adult care.

**Diagnosis coverage and recording requirements**

Type 1/type 2 and other specified diabetes; gestational diabetes linked to MFM; obesity; dyslipidaemia; metabolic syndrome; hypoglycaemia and diabetes complications. Do not classify diabetes solely from device data or silently apply adult rules to pregnancy/children.

**Measurement groups**

| Measurement | Units / representation | Required context |
|---|---|---|
| HbA1c | % or mmol/mol | Assay/date and factors affecting interpretation |
| Glucose | mmol/L or mg/dL | Sensor versus capillary/venous, timestamp and meal context |
| CGM summary | % time; mean; variability | Target range, reporting window, data sufficiency and algorithm |
| Weight/BMI/waist | kg; kg/m²; cm | Measurement dates and longitudinal goals |
| Renal screening | eGFR; urine albumin/creatinine ratio | Equation, units and sample |
| Lipids/BP | mmol/L or mg/dL; mmHg | Analyte, fasting context and technique |
| Insulin/therapy events | product units; timestamps | Device versus reported delivery, formulation and method |

**Equipment and integration requirements**

- Glucose meters, CGM, connected pens and pump portals → authorized manufacturer APIs or validated exports. Dexcom v3 provides developer endpoints; commercial access and latency need confirmation.

- AID/pump control remains within approved systems; imported glucose must never directly trigger unvalidated insulin commands.

**Minimum acceptance scenarios**

- Unit conversion and timezone/daylight changes must preserve original values/times and prevent duplicate CGM points.

- CGM gaps must be visible; incomplete coverage cannot be marketed or interpreted as continuous monitoring.

**Reports and indicators:** Screening completion, patient-specific glycaemic trend, CGM data coverage, hypoglycaemia follow-up and goal review; retain target-range definitions and population context.

**Initial release scope:** Metabolic review, labs, complication register, education and manual device report attachment.

**Advanced scope:** Authorized CGM/pen/pump data and validated standardized summaries.

**Research foundations:** [ADA Standards of Care 2026, section 7: diabetes technology](https://diabetesjournals.org/care/article/49/Supplement_1/S150/163922/7-Diabetes-Technology-Standards-of-Care-in), [Dexcom API v3 endpoint documentation](https://developer.dexcom.com/docs/dexcomv3/endpoint-overview), [HL7 FHIR R4 Observation](https://hl7.org/fhir/R4/observation.html)

## 5. Equipment integration architecture

### Separate five levels of capability

1. **Reference:** attach a signed report or access-controlled link. Fastest initial delivery; little structured data.
2. **Import:** receive structured results, observations, images or case summaries with patient matching and review.
3. **Workflow exchange:** send demographics/orders/worklists and receive status/results, only when supported by both systems.
4. **Validated decision support:** use verified imported information in an approved clinical rule with provenance, review and monitoring.
5. **Device actuation:** change a pump, laser, ventilator, radiation system or robot. This is outside this proposed general EMR integration scope and requires a separate manufacturer-authorized, safety-engineered programme.

Market each actual connector at its verified level. A PDF upload is not a bidirectional device integration, and receipt of a waveform is not permission to interpret it automatically.

### Proposed data path

**Equipment/vendor cloud → protected site gateway/interface engine → immutable inbound record → identity and unit validation → staging/reconciliation queue → authorized clinical record → acknowledgement and audit.**

Keep order dispatch separate from result receipt. Use durable queues, idempotency keys, source identifiers and message sequence/version handling. An HTTP success or transport ACK is not equivalent to clinical acceptance. Store failures, retry policy, dead-letter queue and reconciliation reports. Network separation, authenticated endpoints, least-privilege service accounts and approved remote vendor access are part of implementation, not optional afterthoughts.

| Interface family | Intended use | Validation needed |
|---|---|---|
| HL7 v2 | Patient/encounter messages, orders, laboratory/device results | Exact message/event versions; identifiers; status mapping; ACK/error behaviour; transport/security |
| FHIR | Agreed clinical APIs and structured exchange | Version/profile/terminology; authorization; search and update semantics; provenance; pagination/replay |
| DICOM | Images, modality worklists, selected structured measurements and dose objects | SOP classes; transfer syntaxes; modality/software conformance; study/accession matching |
| DICOMweb | Web-based image query, retrieval and storage | Archive capabilities, authentication, object types, payload size and access controls |
| Analyser-specific protocols | Laboratory instruments via middleware, including ASTM-derived interfaces | Exact instrument manual, host query mode, units, flags, QC and retransmission |
| Manufacturer REST/API/export | CGM, witnessing, remote-device and robotic case data | Contract, intended use, geography, consent, latency, quotas, version changes and supported fields |
| Validated CSV/XML/PDF import | Transitional or low-volume workflows | Format version, signature/source, reconciliation, duplicate protection and manual verification |

### Current documented equipment examples

These are **candidate integration targets**, selected for relevance and recent documentation. This is not a complete market ranking, purchase recommendation or compatibility list.

| Candidate | Verified public evidence | FertPro use to investigate | Unresolved requirement |
|---|---|---|---|
| GE Voluson Expert family | Model/software-specific ultrasound DICOM statements | Patient worklist, images and supported obstetric measurements | Exact installed configuration and SR/worklist coverage |
| Vitrolife eWitness 26.1 | Manufacturer advertises the release and EMR API linkage | Patient/cycle identity and witnessing workflow | API specification, allowed write scope and event coverage |
| Vitrolife EmbryoScope 8 | Manufacturer describes time-lapse/EmbryoViewer workflow | Embryo-linked observations/images and authorised reports | Export rights, format, licensing and local availability |
| Dexcom API v3 | Official endpoints for glucose, events, devices and related data | Consent-based longitudinal glucose review | Commercial access, regional support and operational latency |
| Intuitive da Vinci 5 | Manufacturer describes force feedback and digital capabilities | Approved case summary/video/analytics linkage | Vendor contract; no public general EMR connector verified |

**Concrete latency example:** Dexcom’s documentation states that mobile-app uploads are available through its API after one hour in the US and three hours outside the US, including Japan; receiver USB uploads are described separately. Its original glucose units are mg/dL, and device system/display times have distinct meanings. Therefore this connector should initially support retrospective clinical review, with visible freshness and coverage, rather than real-time emergency monitoring. Recheck terms and latency before implementation. [Dexcom endpoint documentation](https://developer.dexcom.com/docs/dexcomv3/endpoint-overview)

### Vendor questionnaire required before promising compatibility

Obtain model, serial family, software/firmware, country, intended use, interface licence, conformance statement, protocol manual, sample de-identified messages, supported direction, patient matching fields, device clock behaviour, data latency, historical export scope and error handling. Confirm permitted commercial use, patient authorization, security/authentication, data location, retention, maintenance contact and upgrade/change notice. Ask whether the clinic owns its exported records and whether data can be retrieved after contract termination.

Accept a connector only after a test dataset includes normal, abnormal, critical, corrected, cancelled, duplicate, malformed, wrong-patient, unknown-unit, delayed, out-of-order and offline/recovery cases. Record manufacturer/clinic approval and a tested rollback procedure.

## 6. Robotic surgery: a dedicated development programme

### Clinical and data scope

Build one vendor-neutral **RoboticCaseExtension** linked to SurgicalCase, rather than a different surgery record for each robot. It should capture platform/model/software, device and facility IDs, surgeon/assistants, intended/performed procedure, indication, port/setup notes, planned versus actual approach, conversion, docking/undocking/console intervals, instrument identifiers and usage, implant links, energy-device references, events/interruptions and source case ID.

Optional imported data includes manufacturer-defined kinematic summaries, force-related summaries, workflow metrics, images and video. Retain their original names/units, calculation versions and intended purpose. Do not invent a cross-platform “robotic skill score.” Case complexity, patient selection and differing measurement definitions make unadjusted comparisons misleading.

### Platform-specific differences

- **da Vinci 5:** investigate approved exports from the Intuitive digital ecosystem. Force feedback is a manufacturer-described capability, but this research does not establish access to raw force streams, kinematics or a public EMR API. [Intuitive](https://www.intuitive.com/en-us/products-and-services/da-vinci/5)
- **Hugo:** investigate case/media linking through approved Medtronic interfaces. The Touch Surgery ecosystem is explicitly not intended to direct surgery or aid diagnosis/treatment; do not repurpose its analytics beyond that intended use. Its December 2025 US urologic clearance announcement should not be generalized to all procedures or countries. [Hugo platform](https://www.medtronic.com/en-us/healthcare-professionals/specialties/surgical-robotics/hugo-robotic-assisted-surgery.html), [clearance announcement](https://news.medtronic.com/2025-12-03-Medtronic-announces-FDA-clearance-of-Hugo-TM-robotic-assisted-surgery-system-for-urologic-surgical-procedures)
- **Versius Plus:** maintain a device/indication matrix against the applicable authorization. The FDA record confirms a specific clearance; it says nothing about FertPro interoperability. [FDA record](https://www.accessdata.fda.gov/scripts/cdrh/cfdocs/cfpmn/pmn.cfm?ID=K252111)
- **Mako:** distinguish the robotic-arm Mako 4 ecosystem from the handheld Mako RPS platform announced for US total-knee replacement in July 2026. For orthopaedics, prioritize approved plan references, component sizes/lots, implant alignment summaries and postoperative outcomes; do not assume identical data from both systems. [Stryker](https://www.stryker.com/us/en/about/news/2026/stryker-expands-mako-robotics-portfolio.html)

### Integration phases and evidence

| Phase | Deliverable | Required evidence |
|---|---|---|
| R0: structured manual case | Robot fields, implant/instrument records and operative note | Surgeon/theatre-team review and complete simulated case |
| R1: approved report import | Signed vendor case summary attached and matched | Patient/case matching, duplicate/revision tests and provenance |
| R2: structured export connector | Permitted timestamps/events/measurements | Vendor specification, sample data, interface validation and field mapping |
| R3: media and analytics | Protected video/image links, validated operational dashboards | Purpose-specific consent, access control, retention, performance and interpretation review |
| Separate programme | Any device command or control function | Explicit manufacturer authorization and appropriate safety/regulatory engineering; not included in this roadmap |

Video needs separate retention and access rules, consent appropriate to clinical care/training/research, controlled sharing and management of accidental patient/staff identifiers. Do not place full-resolution surgical video in the marketing host or ordinary note fields. Keep device logs, clinician notes and analytics distinct.

The EMR must have no dependency that can halt the robot during an operation. Loss of integration should produce a visible documentation backlog, preserve device autonomy and permit approved local recording followed by reconciled import. A surgical team must review what becomes part of the final clinical record.

## 7. Development template and release gates

Every module should produce the following implementation package before production release:

1. **Charter:** named clinical owner; population; settings; included/excluded workflows; release status; dependency map.
2. **Personas and permissions:** roles, delegated actions, signature authority, guardian/proxy rules and sensitive data.
3. **Workflow diagrams:** entry/exit criteria, transitions, cancellation, exceptional paths, escalation and downtime.
4. **Atomic data dictionary:** field ID, label, definition, datatype, cardinality, source, unit, code/value set, validation, privacy and retention.
5. **Clinical content pack:** forms, report/consent templates, approved rules, reference charts, guideline versions, localization and licences.
6. **Interface contract:** vendor/model/software, level/direction, supported fields, latency, matching, reconciliation and maintenance.
7. **Verification pack:** clinical scenarios, calculation tests, incorrect/missing-data cases, access tests, usability and downtime simulations.
8. **Operations pack:** deployment, migration, training, monitored pilot, rollback, support ownership and change review.
9. **Evidence register:** each marketed feature mapped to a released version, demonstration, test result and supported clinic configuration.

### Example field specification

`mfm.biometry.head_circumference`: decimal; one value per measurement observation, repeatable observations; required subject is fetus ID; unit mm; effective scan time required; source image optional only for manual capture with provenance; method required; chart/version required only when deriving a percentile. Reject incompatible units; allow “not measured” explicitly; flag implausible values for review using locally approved ranges. An amendment creates a corrected observation, retaining the original and author. This illustrates the level of detail required for every measurement group.

### Clinical rule specification

Each rule needs an identifier, purpose, source/date, version, population, inputs/units, missing-data behaviour, output, urgency, responsible recipient, override reason, contraindications/exclusions, test cases and approval. Avoid implementing a threshold from a guideline summary or marketing page. A clinician must be able to inspect why an alert fired. A validated screening instrument does not automatically become a diagnostic algorithm.

### Proposed implementation sequence

| Stage | Scope | Exit gate |
|---|---|---|
| Foundation | Audit existing core; identity, isolation, orders/results, medication basics, consent, audit, backup/restore | Demonstrated patient journey and safe correction/downtime handling |
| First specialty pilot | Gynae and a tightly scoped MFM pathway, or Aesthetics if that is the committed pilot; audit existing IVF/Cryobank functions in parallel within project planning | Named clinical team, approved forms, no unresolved critical defects |
| Diagnostic services | Laboratory, Imaging/Radiology, Pharmacy; choose one real analyser and one real imaging workflow | Live-equipment acceptance and operational ownership |
| Reproductive depth | IVF/embryology, Cryobank, advanced MFM interfaces | Material identity/custody, consent and alarm/fallback evidence |
| Longitudinal programmes | Primary Care, Paediatrics, Geriatrics, Dermatology, Mental Health, Rehabilitation, ID, Diabetes and Cardiology | Programme-specific data, permissions, follow-up and meaningful outcomes |
| High-acuity expansion | Emergency, Neonatology, Surgery, Oncology | Specialist clinical safety review, staffing, downtime and high-risk medication/device validation |
| Advanced robotics | R0–R3 with one partner hospital and one contracted vendor | Manufacturer-permitted exchange, matched records and isolated failure behaviour |

This order is a risk/dependency recommendation, not a fixed commercial schedule. No credible cost or date for all 23 can be established before inspecting existing code, agreeing the pilot scope and obtaining vendor interface quotations. Plan estimates around workflow complexity, clinical review, hardware access, content licences and long-term connector maintenance—not just screen count.

## 8. Marketing handoff rules

The accompanying marketing pack provides one page outline per specialty, audience-specific headlines, proposed feature pillars, a scenario and calls to action. Keep a visible **planned / pilot / available** label near each offering. Use “planned scope includes” until release evidence exists. “Available” requires a released build and clinical acceptance for the stated configuration. “Integrated with [device]” additionally requires tested model/software compatibility and permission to use any vendor branding.

Do not claim all specialties are clinically complete because their catalogue entries exist. Do not claim improved pregnancy rates, reduced mortality, guaranteed diagnosis, autonomous treatment, universal equipment compatibility or certification without appropriate evidence. Use synthetic demonstration records; do not include patient data in sales collateral.

The public positioning can be: **FertPro EMR is developing a connected clinical platform, with specialty-specific workspaces built around one patient-care foundation. Explore the planned scope for your service and discuss a clinically validated rollout.**
