FERTPRO PRIMARY CARE
Planned specialty scope · Availability subject to clinical validation

A clearer picture of every patient, across every visit.

Longitudinal care, prevention and chronic disease follow-up.

THE CLINICAL JOURNEY

Designed around the way
your specialty works.

Longitudinal person record, problem-oriented encounter, household links and referral episode.

01

Front desk reconciles identity, contact preferences, payer and caregiver relationships; nurses capture triage and screening.

02

Clinician opens an encounter organized by concern, history, examination, differential, confirmed conditions and plan; chronic problems persist independently of the visit.

03

Preventive-care recalls, immunization administration with batch records, chronic disease follow-up, medication reconciliation and referral tracking share one timeline.

04

Close the loop on ordered tests, reviewed results, patient communication and missed appointments; distinguish a referral sent from one actually completed.

Diagnosis coverage and clinical records

Hypertension; diabetes; asthma/COPD; acute respiratory and urinary infections; musculoskeletal complaints; anaemia; obesity; multimorbidity. Capture suspected versus confirmed status, onset, severity, evidence, resolution and responsible clinician; symptom codes remain valid before diagnosis.

Equipment and integration plans
  • Connected BP monitors, scales and pulse oximeters → observations through approved gateway or validated import; manual entry always retains provenance.
  • Spirometers, ECG and point-of-care analysers → measurements, waveform/report and QC status through manufacturer interface; referral labs → order/result messaging.

Connections require an approved interface and testing against the installed manufacturer, model and software. Listed equipment is a candidate integration target.

Reporting and meaningful outcomes

Referral completion = completed referrals / eligible referrals due; result-review turnaround from verified result receipt to clinician acknowledgement; chronic review attendance with declared due-date window.

Development scope and clinical validation

Initial scope

Encounter template, conditions, vitals, medicines, orders/results, recalls and referrals.

Advanced scope

Home monitoring, population registries and patient-reported outcomes after basic identity and result routing work.

Example acceptance scenarios

  • Duplicate or merged identity must not silently merge households or disclose one family member’s record.
  • A cancelled test stays distinct from a normal result; unreviewed abnormal results remain on an accountable worklist.
