Designed around the way
your specialty works.
Comprehensive geriatric assessment linked to patient goals, function, support network and evolving care plan.
Assess physical illness, cognition/mood, mobility, function, nutrition, medicines, social support and living environment.
Document frailty, falls, continence, sensory impairment, delirium risk and caregiver concerns without reducing the person to a score.
Coordinate medication review, rehabilitation, social-care referrals, capacity assessment, advance preferences and agreed goals.
Support discharge planning, home/care-facility transitions and periodic reassessment with accessible communication.
Diagnosis coverage and clinical records
Multimorbidity, frailty syndromes, dementia, delirium, falls, osteoporosis, sarcopenia, incontinence and malnutrition. Differentiate chronic cognitive impairment from acute change; record functional consequences and reversible contributors.
Measurements that give the record meaning
| Measurement | Units / representation | Clinical context |
|---|---|---|
| Function | ADL/IADL instrument scores | Baseline, current state, assistance and respondent |
| Frailty | named scale score | Version, assessor and intended population |
| Mobility | m/s; seconds | Gait distance, walking aid and test method |
| Orthostatic BP | mmHg | Position and elapsed standing time |
| Nutrition | kg; % change; instrument score | Time window and intake context |
| Cognition/mood | instrument scores | Language, education adjustments and licensing |
| Medication burden | count; coded risks | Active medicines, review date and indication |
Equipment and integration plans
- Home BP/weight devices, fall sensors and activity monitors → consented trends rather than proof of falls or independence.
- Rehabilitation and care-provider systems → functional assessments and transition summaries with role-limited caregiver access.
Connections require an approved interface and testing against the installed manufacturer, model and software. Listed equipment is a candidate integration target.
Explore equipment & roboticsReporting and meaningful outcomes
Care-plan goal review, medication-review completion, falls with exposure period, functional change and transition follow-up; patient preference remains visible alongside utilization.
Development scope and clinical validation
Initial scope
CGA workspace, medication review, goals, caregiver roles and transitions.
Advanced scope
Home-monitoring and community-service exchange with consent controls.
Example acceptance scenarios
- A proxy’s reported history must be distinguishable from the patient’s account; capacity is decision/time specific.
- An acute confusion flag must not automatically overwrite an established dementia diagnosis or imply incapacity.
